29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face. a) Jobsite: North wall of excavation, by sewer inlet on corner of W Courtland and N 16th Streets - The Employer did not provide employees protection from loose soil on the face of the excavation. The employee suffered an injury when a small portion of wet soil fell off the face of the excavation and struck him on the shoulder. Noted on or about 9/10/18.
