OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees operated a natural gas fired oven while exposed to slip, trip, fall, burn and explosion hazards:    a) Kitchen, 1424 South Main Street, Mansfield, PA - Employees operated a castered Blodgett Oven Company Model XR8G/AB Mini Rack Oven (Serial No. 092412DJ0927) without a restraint (safety device) to prevent strain from being imparted onto the natural gas supply line and connections to the appliance exposing the employees to burn and explosion hazards, on or about September 10, 2018.    b) Kitchen, 1424 South Main Street, Mansfield, PA - Employees operated a castered Blodgett Oven Company Model XR8G/AB Mini Rack Oven (Serial No. 092412DJ0927) with a water drain directly beneath the oven exposing the employees to slip, trip, and fall hazards, on or about September 10, 2018.    In accordance with 29 CFR 1903.19, abatement documentation and certification is required for this violation.    The Swaminarayan Bapa Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH ACT of 1970 Section (5)(a)(1), which was contained in OSHA inspection number 1317627, citation number 1, item number 1 and was affirmed as a final order on July 3, 2018, with respect to a workplace located at 1424 South Main Street, Mansfield, PA 16933.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Kitchen, 1424 South Main Street, Mansfield, PA - The energized power cord of the Blodgett Oven Company Model XR8G/AB Mini Rack Oven (Serial No. 092412DJ0927) lacked strain relief and had been abraided by the non-metallic cable clamp type connector that was not being used in accordance with its listing or labeling exposing employees to a contact with electrical current hazard, on or about September 10, 2018.    In accordance with 29 CFR 1903.19, abatement documentation and certification is required for this violation.    The Swaminarayan Bapa Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1317627, citation number 1, item number 2a and was affirmed as a final order on July 3, 2018, with respect to a workplace located at 1424 South Main Street, Mansfield, PA 16933.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Kitchen, 1424 South Main Street, Mansfield, PA - Employees used the Blodgett Oven Company Model XR8G/AB Mini Rack Oven (Serial No. 092412DJ0927) that was energized by an extension cord used in lieu of fixed wiring exposing employees to a contact with electrical current hazard, on or about September 10, 2018.  In accordance with 29 CFR 1903.19, abatement documentation and certification is required for this violation.  The Swaminarayan Bapa Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1317627, citation number 1, item number 2b and was affirmed as a final order on July 3, 2018, with respect to a workplace located at 1424 South Main Street, Mansfield, PA 16933.

In accordance with 29 CFR 1903.19, abatement documentation and certification is required for this violation.    The Swaminarayan Bapa Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.263(l)(9)(ii), which was contained in OSHA inspection number 1317627, citation number 2, item number 2 and was affirmed as a final order on July 3, 2018, with respect to a workplace located at 1424 South Main Street, Mansfield, PA 16933.
