29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a)  On or about July 11, 2018, the employer did not protect employees working on the ground below roofing operations from overhead falling object hazards by ensuring the employees were wearing hard hats. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about July 11, 2018, the employer did not did not ensure employees were protected from fall hazards of 10 feet and greater while engaged in residential re-roofing operations.  Abatement Note:  Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").  Carmelo Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1216967, citation number 2, item number 1 and was affirmed as a final order on June 8, 2017, with respect to a workplace located at 16185 S Golden Rd, Golden, CO 80401.   Carmelo Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1173168, citation number 2, item number 1 and was affirmed as a final order on October 25, 2016, with respect to a workplace located at 7 Burgundy Ln, Highlands Ranch, CO 80126.  Carmelo Construction, LLC. dba Carmelo Gutierrez Roofing, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 979735, citation number 1, item number 1 and was affirmed as a final order on July 23, 2014, with respect to a workplace located at 4161 S Laredo Way, Aurora, CO 80013.
