29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall arrest systems:  (a) 21697 240th Ave, Lacona, IA 50139; 3467 228th Ave, Hartford, IA 50118; 3013 228th Ave, Hartford, IA 50118; 2982 220th Ave, Hartford, IA 50118; 8584 180th Ave, Carlisle, IA 50047 - Employees were potentially  exposed to fall hazards when accessing the center and eave roof hatches and walking on top of the storage bin roofs throughout several service locations. Estimated heights ranged from 12 feet to 24 feet. These conditions were noted on or about 11/30/2018.

The south emergency exit door was locked from the inside. This condition was noted on or about 9/5/2018.
