29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) All Packaging Company 14806 E. 33rd Place, Aurora, CO 80011: On and before September 11, 2018, the employer did not ensure the points of operation were properly guarded to prevent employees from having any part of their body in the danger zone during operation in that all of the interlocked guards on the two Bobst Expert Fold 110 machines were defeated and the manufacturer provided guards were either broken or removed resulting in the machines being operated in continuous run mode without effective guards in place. This condition exposed the employees to amputation hazards.
