29 CFR 1910.28(b)(3)(iv): (iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  On or about September 11, 2018, employees of Concrete Industries were exposed to fall hazards on the mezzanine behind the concrete pipe mixer while access the mezzanine from a fixed ladder. The access point from the fixed ladder was too wide and a self closing gate or offset had not been installed.

On or about September 5, 2018 the crane operator suspending Catch Basins Forms in the Concrete forms area by the concrete pipe mixer left the controls of the crane to help attach the lifting bolts to the Catch Basin Form.

29 CFR 1910.184(c)(7): Slings shall be padded or protected from the sharp edges of their loads.    On or about September 11, 2018, employees hoisting concrete forms in the front concrete form area were using alloy steel chains that had been wrapped around pieces of rebar which created a curving radius that was too tight on the chain link exposing the link to a sharp edge on the rebar.

29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    On or about September 11, 2018 Concrete Industries did not have certifications of forklift operator evaluations at their facility.
