29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. On or about July 29, 2019, and employee of Concrete Industries was observed driving a Hyster 100 forklift through the plant with a concrete hopper on the forks of the lift. The hopper impaired the employee's forward view and he was not travelling in reverse to mitigate the impairment.

29 CFR 1910.254(d)(9)(i): The operator should report any equipment defect or safety hazard to his supervisor and the use of the equipment shall be discontinued until its safety has been assured. Repairs shall be made only by qualified personnel. On or about July 29, 2019, employees were exposed to potential shock hazards when a Lincoln Electric AC-225 Arc Welder showed signs of defects and repairs that had been completed by un-qualified personnel.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions. [55 FR 13709, Apr. 11, 1990; 61 FR 9227, March 7, 1996; 70 FR 53929, Sept. 13, 2005] On or about July 29, 2019, employees were exposed to shock hazard when the electrode cable of an Lincoln Electric AC 180 Amp Arc Welder in the Welding Shop had been repaired with duct tape.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about July 29, 2019, employees were exposed to shock hazards when the insulation covering the internal wiring of the power input cable of a Lincoln Electric AC 180 Amp Arc Welder was pulled from head of the power cable exposing the internal wiring.
