29 CFR 1917.26(f): A US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees' work exposed them to the hazard of drowning.    Sause Bros terminal:  On or about September 11, 2018 and times prior thereto, life rings only had thirty feet of rope attached.  This exposed employees to delays in a rescue if they had fall into the river.

Namakani barge:  On or about September 11, 2018 and at times prior thereto, employees were standing within three feet of the barge edge with a fall hazard into the water.

(a)  Sause Bros terminal:  On or about September 11, 2018 and times prior thereto, the gangway to the Hana barge was not properly trimmed.  This exposed employees to having to reach a fixed ladder exposing employees to a fall hazard.   (b)  Sause Bros  terminal:  On or about September 11, 2018 and times prior thereto, the gangway to the Siuslaw barge was not properly trimmed.  This exposed employees to having to put their bodies in between the gangway and a fixed ladder exposing employees to caught between hazard.

(a)  Siuslaw Barge:  On or about September 11, 2018 and times prior thereto, employees were exposed to a fall into the river due to there being no protection when employees accessed the end of the gangway and fixed later on the barge.  (b)  Hana Barge:  On or about September 11, 2018 and times prior thereto, employees were exposed to a fall into the river due to there being no protection when employees accessed the end of the gangway and fixed later on the barge.

Namakani barge:  On or about September 11, 2018 and at times prior thereto, an employee was driving a forklift and was not wearing a seatbelt during the off load operations.  This exposed employees struck-by hazards event of a tipover accident or driver being ejected from the vehicle.
