OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards while operating a mini skid steer.   On or about September 13, 2018 an employee operating a Vermeer S725 TX skid steer received fatal injuries when the Bradco scrap grapple used to lift sheets of plywood fell from a raised position, striking the operator. The skid steer's attachment pins that secured the grapple to the skid steer were not properly in place.
