29 CFR  1910.28(b)(13)(i): (13) Work on low-slope roofs. (i) When work is performed less than 6 feet (1.6 m) from the roof edge, the employer must ensure each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system.   a.  On or about October 14, 2020, located at 1233 North Columbus Street, Lancaster, Ohio:  the employer did not ensure that an employee was protected from falling to the concrete ground level while throwing brush off the roof top.  The employee walked to the edge  of the roof top to conduct the task.  The employee was  exposed to a fall distance of approximately 16 feet 9 inches from the roof top edge to the concrete ground level below.

29 CFR  1910.30(a)(1):(a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  a.  On or about October 14, 2020, located at 1233 North Columbus Street, Lancaster, Ohio:  the employer did not ensure that an employee was trained to use the personal fall protection system while throwing brush and branches off the roof top to the chipper machine below.  The employee had walked and looked over the edge of the roof top to conduct the task.  The employee was  exposed to a fall distance of approximately 16 feet 9 inches from the roof top edge to the concrete ground level below without being properly trained.

29 CFR  1910.67(c)(2)(iv):Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.  a.  On or about October 14, 2020, located at 1233 North Columbus Street, Lancaster, Ohio:  the employer did not ensure that an employee stood firmly on the floor of the basket, nor did the employer ensure that the employee did not sit on the edge of the aerial lift basket.  An employee had sat on top of the aerial lift basket and was exposed to a fall hazard above the roof top.

29 CFR  1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.  a.  On or about October 14, 2020, located at 1233 North Columbus Street, Lancaster, Ohio:  the employer did not ensure that an employee in the aerial lift basket wore the personal fall arrest or travel restraint system.  The employee had operated the aerial lift basket to the top of the roof and sat on  top of the basket without using the personal fall arrest or travel restraint system on throughout the process.
