29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jose Perez located at 2482 Lake Vista Drive, Broomfield, CO 80023: On and Preceding 15 September 2018, the exposing employer did not ensure that employees engaged in residential roofing activities up to 25 feet above the lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems. Two employees were removing shingles and trim on the roof of a two-story residential home without the use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jose Perez located at 2482 Lake Vista Drive, Broomfield, CO 80023: The exposing employer did not ensure that ladders used to access an upper landing surface had side rails extended at least three feet above the upper landing surface. Employees were using an extension ladder to access the roof with approximately 1'6" extending above the landing surface. This exposed employees to falls from height of approximately 10 feet.
