29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about July 19, 2018, on the West side of a newly built commercial building, one employee was exposed to a fall hazard exceeding 10 feet when performing work from a wooden pallet placed on the forks of a JLG forklift, Model 6042, without the forklift being designed for such use.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about July 19, 2018, on the West side of a commercial building being constructed when an employee was exposed to a fall hazard while working from the second level of a scaffold, installing metal siding and metal fascia, without a fall protection system in place.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about July 19, 2018, and times prior thereto, on the low slope roof of a newly constructed commercial building, an employee was exposed to a fall hazard exceeding six feet when performing roofing work without a fall protection system.
