29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about March 10, 2021, employees were installing paper on the roof of the church from heights above 6 feet without the use of fall protection, exposing employees to a fall hazard.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about March 10, 2021, employees were not properly trained in recognizing and minimizing fall hazards associated installing a roof at heights above 6 feet; exposing employees to a fall hazard.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about March 10, 2021, employees were accessing the roof of the church by means of a ladder that did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard.
