29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: On or about September 17, 2018, an employee who was operating a "walkie rider" powered industrial vehicle, was involved in an accident when he was struck by the overhead door leading into the raw meat cooler, lost control of the vehicle, and was knocked off of the vehicle, hitting his side on a meat storage rack. The employee was not retrained after this accident occurred.
