29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience:  Location: Refrigeration room  On or about September 25, 2018, the Scott Instruments, QuadScan II permanent ammonia detection sensor was out of calibration.

29 CFR 1910.120(q)(3)(iv): Employees engaged in emergency response and exposed to hazardous substances presenting an inhalation hazard or potential inhalation hazard, did not wear positive-pressure self-contained breathing apparatus until such time that the individual in charge of the incident command system determined through the use of air monitoring that a decreased level of respiratory protection would not result in hazardous exposures to employees:  Location: 2nd floor, chocolate factory  On or about September 11, 2018, employees that responded to the facility's ammonia leak within the building did not use a hand-held ammonia gas meter to test the atmosphere and did not wear respiratory protection prior to entering the room where the leak had occurred.
