29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a) Warehouse of 919 Heald Street - On or about 11 SEP 2018, the employer provided employees with a front end attachment, manufactured by a third party, to use with a TCM by Unicarrier Brand sit down forklift without markings to show the cumulative weight of the truck and the front attachment with a load laterally centered at maximum elevation.
