On or about 12/16/2019, in the pillow production lines at 1720 GA Highway 138, Conyers, GA: Hopper machines # 1 and 2 lockout tagout procedure did not specifically identify each 240 volt power source, mechanical, gravity, and pneumatic 120 psi sources, exposing employee to caught-in and amputation hazards.  On or about 12/16/2019, in the pillow production lines at 1720 GA Highway 138, Conyers, GA: Lapper machines # 1 and 2 lockout tagout procedure did not specifically identify each 240 volt power source, mechanical and pneumatic 120 psi sources, exposing employee to caught-in and amputation hazards. 29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  On or about 12/16/2019, in the pillow production lines one and two at 1720 GA Highway 138, Conyers, GA: Employees engaged in service and maintenance activities on the two picker equipment while clearing, unjamming, or troubleshooting were exposed to amputation and caught-in hazards, in that they did not apply a lock and or tag device on the equipment energy sources.   Pickers lines one and two with electrical 240 volts and mechanical moving energy sources.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  On or about 12/16/2019, at 1720 GA Highway 138, Conyers, GA: Where employee(s) operated a Yale and Toyota powered industrial truck with defects and deficiencies such as but not limited to inadequate parking brakes to prevent the lifts from moving unattended and the backup alarm not working.  The employer failed to remove the powered industrial truck from service until the repairs could be made.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: The guard were so installed that they offered accident hazards in themselves.  On or about 12/16/2019, at the two (2) Picker equipment at 1720 GA Highway 138, Conyers, GA: Employees performing servicing and maintenance activities on the pickers were exposed to amputation and caught-in hazards, in that the employer failed to ensure the guards were secured in place to prevent movement, removal, and secured to prevent easy access from unauthorized employees to the moving and rotating parts.
