29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings, where the outside bottom edge of the wall opening was 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) 100 Kenwood Road, Albany, NY, on September 18, 2018, and at times prior, a gated opening (measuring 42" x 41") in the wall of the South Stairway in Building 1 on the 4th floor was not secured, creating a potential 20 foot fall hazard.
