29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes and fittings were not effectively closed. a) 100 Kenwood Road, Albany, NY, Bldg. 3, 3rd floor, on September 18, 2018, and at times prior, a temporary circuit breaker panel box had a knockout missing on the top. b) 100 Kenwood Road, Albany, NY, Bldg. 2 "C" wing 1st floor, on September 18, 2018, and at times prior, a temporary power outlet box had two knockouts missing on one side.

29 CFR 1926.405(d): Switchboards that have any exposed live parts were not located in permanently dry locations and accessible only to qualified persons. Panelboards were not mounted in cabinets, cutout boxes, or enclosures designed the purpose and were not dead front. Exposed blades of knife switches were not dead when open: a) 100 Kenwood Road, Albany, NY, on September 18, 2018, and at times prior, electric panel box "3A" on the third floor of building 1 did not have an inside cover for the energized parts which exposing employees to energized bus bars.
