29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:    a) Facility: On September 19, 2018 and at times prior thereto Woodgrain Millwork employee(s) operated powered industrial trucks to off-load machinery of various sizes, shapes and weight from a flatbed truck.  While the Woodgrain Millwork employee(s) used the manufactured installed fork pockets on the equipment, the load(s) were not stable or positively secured as they were lifted, moved, and lowered to the ground.  This exposed employees to the hazard of an unstable load.        b) Facility: On October 19, 2018 and at times prior thereto when handling larger units of wood, the roll and dump system/attachment used on some powered industrial trucks did not hold the load steady, allowing it to "drift" to one side or the other.  This included, but may not be limited to Yale forklift number 14.          Note) Certification and documentation are required for this item.
