29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) East end of Test Scoring Room #221: A 120 volt a relocatable power tap designed and listed to provide power to high concentrations of low powered loads such as computers, printers, radios, etc. was instead used to provide power to high powered equipment such as a Woods full sized refrigerator which was plugged into the power tap and not a nearby wall mounted outlet.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable trays, cable armor, cable sheath, enclosures, frames, fittings, and noncurrent-carrying parts that are to serve as grounding conductors, with or without the use of supplementary equipment grounding conductors, were not effectively bonded where necessary to ensure electrical continuity and did not have the capacity to conduct safely any fault current likely to have been imposed on them: a) Southeast wall of the Com Room: A 120 volt flexible electrical cable providing power to a cord connected to a quad box had pulled away from the plug-end exposing the insulated wires ; It was not effectively bonded. ABATEMENT CERTIFICATION IS REQUIRED
