On or about September 19, 2018: Employees working on Line 12 and Line 13 are exposed to burn hazards where they are able to place parts of their body into the danger zone of the open flame of the Flynn Flame Manager CC120 P/N 090833. The employer did not ensure that the open flame was guarded by one or more methods of primary machine guarding.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. On or about September 19, 2019: Employees working in the production area and cleaning the Deco printer plates are exposed to chemicals such as blanket wash (solvent blend), and alcohol. The small containers of solvent and alcohol did not have warning words or pictograms to indicate their hazards.
