Vazquez Roofing worksite located Lot 82 Tullip Trail, Appleton Wisconsin:  On or about September 18, the employer did not ensure employees were protected from falls while installing shingles on the two story residential structure. The employees were exposed to a fall distance of approximately 25 feet from a steep slope roof.
