29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:   a) On or about September 25, 2018, employees performing welding in the shop were provided half face tight-fitting P-100 respirators for voluntary use. The employer did not establish and implement a written respirator program that includes at a minimum, the following elements:  (i) Medical evaluations to ensure employees are medically able to use a respirator.  (ii) Procedures for cleaning, storing, and maintaining the respirator to prevent a health hazard to the user.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.   LB Metals, LLC, Harvey, Illinois - On or about September 25, 2018, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was incorrectly filled for the calendar year 2018.  a) On or about June 12, 2018, a recordable injury occurred when an employee received a laceration to the left hand when using a grinder to clean an area around a channel frame, resulting in 26 days of restricted duty. Not all portions of the OSHA Form 301 were completed, including the information requested in Nos. 14, 15 and 17.  b) On or about June 16, 2018, a recordable injury occurred when an employee fractured a left hand finger after a steel part slipped when stacking racks, resulting in 64 days of lost time (days away from work).  Not all portions of the OSHA Form 301 were completed, including the information requested in Nos. 14, 15 and 17.  c) On or about August 3, 2018, a recordable injury occurred when an employee flipping small parts experienced right elbow pain, resulting in 35 days of restricted duty. Not all portions of the OSHA Form 301 were completed, including the information requested in Nos. 14, 15 and 17.  d) On or about August 16, 2018, a recordable injury occurred when an employee fractured the left foot when a steel part being moved by a jib crane fell and slid onto the foot, resulting in 25 days of lost time (days away from work). Not all portions of the OSHA Form 301 were completed, including the information requested in Nos. 14, 15 and 17.  e) On or about August 20, 2018, a recordable injury occurred when an employee received a laceration to a left hand finger when a steel part being moved fell onto the finger, resulting in 17 days of restricted duty.  Not all portions of the OSHA Form 301 were completed, including the information requested in Nos. 14, 15 and 17.
