29 CFR 1910.147(c)(1) The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury: On or about September 25, 2018, the employer had not established a lock-out/tag-out program for the facility for such machines as, but not limited to the frozen pita line in use on the production floor.

29 CFR 1910.263(j)(1)(iii): The cover over the knife head of reciprocating-blade slicers shall be provided with an interlocking arrangement so that the machine cannot operate unless the cover is in place. On or about September 25, 2018, the hinged door guard for the frozen pita machine, which allows access to the dough slicing process, was not interlocked as required.
