29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o).  a)    Sail loft: On or about 11/08/2018, the lower pulley of (3) three older model Singer sewing machines were not provided with a guard to protect employees from an ingoing nip point as a result employees were exposed to a possible injury.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:(a)  a)    Sail loft: On or about 11/08/2018, the vending machines were not used according to its listing and labeling in that the GFCI plug to the vending machines were plugged into an extension cord.     b)    Sail Loft: On or about 11/08/2018, the employer provided a Tappan microwave to be used by employees that was labeled for household use only.      c)    Sail loft: On or about 11/08/2018, a surge strip was not used according to its listing and labeling in that the male attachment plug was plugged into an extension cord.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a)    Sail loft: On or about 11/08/2018, the black hurricane fan was not visually inspected before use in that the outer jacket to the flexible cord was pulled away from its housing thereby caused tension on the internal connectors.     b)    Sail loft: On or about 11/08/2018, the vacuum cleaner used by employees to blow air into containments was not visually inspected before use in that the outer jacket to the flexible was damaged and had pulled away from its housing thereby caused tension on the internal connectors.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
