29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)    (a) 3315 Market Street, Philadelphia, PA 19104 - Korman Center Rooftop:  Employees were not provided information/training on the hazardous chemicals in their work areas.  Employees are exposed to hazardous chemicals, including but not limited to Stoddard Solvent, Asphaltum, Asphalt (petroleum), etc. contained in PA-917 LS Primer.      Condition noted on/about May 9, 2018.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    (a) 3315 Market Street, Philadelphia, PA 19104 - Korman Center Rooftop: Employees were exposed to falls of approximately 33 feet in that the employer did not provide a fall protection system for employees while disposing various materials into a shoot from a platform on a scaffold system.       Condition observed on/about May 9, 2018.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a) 3315 Market Street, Philadelphia, PA 19104 - Korman Center Rooftop: Employees were exposed to falls of approximately 33 feet.  Employees were conducting roof work on a flat roof and were not protected by use of a monitoring system.       Condition observed on/about May 9, 2018.
