29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    On and about September 25, 2018, employees were exposed to struck by hazards while erecting an engineered steel building when the employer did not train each employee to recognize and avoid or correct the hazards they were exposed to, or to understand and follow the OSHA Standards that applied to their scope of work, in that:    A) Employees were working in or near the fall zone of suspended loads;    B) Employees were walking under the elevated boom of the rough terrain forklift;    C) Employees were not trained to select, inspect, and use appropriate rigging equipment and methods;    D) Forklift operator training did not include the proper use of attachments.

Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace.    29 CFR 1910.178(l)(3)(i)(G): Fork and attachment adaptation, operation, and use limitations;    On and about September 25, 2018, employees were exposed to struck by hazards when the employer did not train the operator how to select and use approved attachments, in that:    A) Steel building components were lifted using slings draped over the forklift tines;    B) An unlabeled, shop fabricated spreader bar was provided for lifting bundles of decking material.

29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service.    On September 24, 2018, employees were exposed to struck by hazards while handling steel building components using rigging equipment that had not been inspected and damaged equipment was being used, in that:    A) Two synthetic slings had cuts and abrasion with red indicator threads visible;    B) Untagged and unrated carbon steel chain, hooks, and links were used for lifting.

29 CFR 1926.251(a)(2)(i): Has permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load;    On September 25, 2018, employees were exposed to struck by hazards while handling steel building components with rigging equipment that was not tagged or marked with working load limits, in that carbon steel chains and connecting links were not stamped with grade markings and the sling assemblies were not tagged with working load information.

29 CFR 1926.251(a)(4): Special custom design grabs, hooks, clamps, or other lifting accessories, for such units as modular panels, prefabricated structures and similar materials, shall be marked to indicate the safe working loads and shall be proof-tested prior to use to 125 percent of their rated load.    On September 25, 2018, employees were exposed to struck by hazards while using a shop fabricated spreader bar for lifting bundles of steel decking with a JLG 8042 RT forklift when the spreader was not marked with the safe working load capacity or proof tested to determine its ability to safely handle the maximum load.
