Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a. On or about September 25, 2018, at the above addressed jobsite, employees installing roof vents, approximately thirty (30) feet above the ground, were not protected from falling.    b. On or about September 25, 2018, at the above addressed jobsite, employees installing roof vents on the garage, approximately nine (9) feet above the ground, were not protected from falling.
