29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: At a site located at 820 N Dakota Drive, Independence, MO: An Employee engaged in power washing activities was exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals. The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work. Hazardous chemicals used at the site included but were not limited to gasoline and oil.

29 CFR 1910.1200(h)(1): At this establishment, the employer had not provided employees with effective information and training, to include pictograms, on hazardous chemicals in their work area as specified in the Revised 2012 Hazard Communication Standard. At a site located at 820 N Dakota Drive, Independence, MO: An Employee engaged in power washing activities was exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals. Employees had not been provided with training or information regarding the hazardous chemicals with which they work. Hazardous chemicals such as gasoline and oil were used in the pressure washer.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable injury that could be prevented by such equipment. At a jobsite located at 820 N Dakota Drive, Independence, MO: Employees were exposed to struck-by hazards. An employee was pressure washing from an aerial lift without the use of any form of eye protection. An Employee was exposed to struck-by hazards from debris flying off of a building when eye protection was not provided or worn at the time of the inspection.
