29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly, and maintained in a safe condition.  a) At the facility:  The floors throughout the facility had cracks varying in sizes, exposing employees to slip, trip, and fall hazards, as observed on or about September 26, 2018.  b) The dock plate on door bay #25 had a tear which resulted in exposed sharp edges, exposing employees to a slip, trip and fall hazard, observed on or about September 26, 2018
