29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested and whenever a different respirator facepiece (size, style, model or make) was used: (Construction Reference 1926.103) (a) Gym Building -1700 South Lincoln Ave., Lebanon, PA - On or about September 26, 2018, an employee was observed wearing a full-face respirator and the employee was not annually fit tested. The employee's last documented fit test was conducted on April 15, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assessed and limited the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section. (a) Gym Building -1700 South Lincoln Ave., Lebanon, PA - On or about September 26, 2018, an employee used a Husqvarna K3000 hand-held saw to cut multiple brick layers for a large ventilation opening without using an integrated water delivery system to continuously feed water to the blade as required by Table 1, and the employer did not assess the silica exposure of each employee. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
