The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a. On or before September 26, 2018, the exit route in the in the D Dock E area of the warehouse was blocked. b. On or before September 26, 2018, the exit route in the in the D Dock K area of the warehouse was blocked.

Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a. On or about September 26, 2018, tent posts and support hardware were stored with the ends protruding into passageways, thereby exposing employees to struck-by or other hazards.
