29 CFR 1926.20(b)(2):  It shall be the responsibility of the employer to initiate and maintain programs as may be necessary to comply with accident prevention; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On or about September 26, 2018 at 605 Washington Street in Manitowoc, the employer did not conduct adequate safety inspections prior to employees working on a steel beam structure at greater than forty feet above surface level.  Employee climbing boom lift basket guardrails, fall arrest system installed with a greater than six feet free fall and used with two snap-hooks connected to body harness D-ring, body harness D-ring positioned below shoulder blades, and two tie-off adapter ends connected to the lanyard snap-hook.

29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.    On September 26, 2018 at 605 Washington Street in Manitowoc, an employee was climbing out/in of an aerial lift basket to access/egress the top of a steel beam at greater than forty feet above surface level.

29 CFR 1926.502(d)(6)(iii):  Unless the snap-hook is a locking type and designed for the following connections, snap-hooks shall not be engaged to a D-ring to which another snap-hook or other connector is attached; reference CFR 1926.760(d)(1) Steel Erection Subpart R.    On September 26, 2018 at 605 Washington Street in Manitowoc, an employee was accessing/egressing a structure and climbing and working at the top of a vertical beam at greater than forty feet above surface level using a personal fall arrest system with two snap-hooks connected to the body harness D-ring; a double legged lanyard snap-hook and a D-ring extension snap-hook.

29 CFR 1926.502(d)(6)(v):  Unless the snap-hook is a locking type and designed for the following connections, snap-hooks shall not be engaged to any object which is incompatibly shaped or dimensioned in relation to the snap-hook such that unintentional disengagement could occur by the connected object being able to depress the snap-hook keeper and release itself.    a)  On September 26, 2018 at 605 Washington Street in Manitowoc, an employee who was working on a vertical beam greater than forty feet above surface level had anchored his personal fall arrest system with the lanyard snap-hook connected to two loop ends of a cable tie-off adapter which was basket wrapped around a chevron beam.    b)  On or about September 26, 2018 at 605 Washington Street in Manitowoc, an employee who was accessing a steel structure greater than forty feet above surface level had anchored his personal fall arrest system with the lanyard snap-hook connected to two ring ends of a wire rope tie-off adapter which was basket wrapped around the walking surface beam.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest system when stopping a fall shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; reference CFR 1926.760(d)(1) Steel Erection.    a)  On September 26, 2018 at 605 Washington Street in Manitowoc, an employee was accessing a horizontal beam from a boom lift basket at greater than forty feet above surface level using a personal fall arrest system installed with a greater than six feet free fall.  Eight feet long wire rope tie-off adapter basket wrapped on access point beam and connected to stretchable lanyard.    b)  On September 26, 2018 at 605 Washington Street in Manitowoc, an employee was working at the top of a vertical beam at greater than forty feet above surface level using a personal fall arrest system installed with a greater than six feet free fall.  Four feet long, orange colored, loop end cable tie-off adapter basket wrapped on a chevron beam and connected to a stretchable lanyard.

On September 26, 2018 at 605 Washington Street in Manitowoc, an employee was working on a steel structure at greater than forty feet above surface level using a personal fall arrest system with the body harness D-ring positioned below the shoulder blades.
