29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At the residential construction site located at 110 Jenny Street, Raymore, Missouri:  Employees were exposed to slip, trip and fall hazards due to debris strewn about the jobsite.  Debris was not cleaned up on a regular basis.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    At the residential construction site located at 110 Jenny Street, Raymore, Missouri:  Employees were exposed to fall hazards up to 17 feet from eave of structure to ground level while conducting framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   At the residential construction site located at 110 Jenny Street, Raymore, Missouri:  Employees were exposed to fall hazards in that the portable ladder used to access the upper landing surface did not extend 3 feet above the landing surface.
