29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical or radiation agents:  At the residential construction located at 110 Jenny Street, Lot 40 in Raymore, Missouri:  Employee was exposed to struck-by hazards.  Employee engaged in framing activities, using a pneumatic nail gun, was exposed to struck-by hazards in that eye protection was not provided.

The supported scaffold was not fully planked exposing employee to an incomplete walking/working surface and subsequent fall hazard of up to 12 feet.

The supported scaffold  frames were not provided with base plates and mud sills or or other adequate foundation.  Scaffold frames were either supported by base plates or mud sills but not both, exposing employees to a scaffold collapse.  The fall hazard is up to 12 feet.
