29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) JD Construction & Framing Inc.  located at 3660 W 25th Street, Greeley, CO 80634:  On and preceding October 1, 2018, the employer did not ensure that the extension ladder side rails extended at least 3 feet above the upper landing surface being accessed.  Four framers were using an extension ladder to access the second floor subflooring that was flush with the landing surface when the length of the ladder would have allowed at least three feet of extension.  This exposed employees to falls from height.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) JD Construction & Framing Inc.  located at 3660 W 25th Street, Greeley, CO 80634:  On and preceding October 1, 2018, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by a guardrail system, safety net system, or personal fall arrest system.  Employees were working on the subfloor of the second floor without the use of fall protection.  Fall hazards included perimeter fall hazards and stairway holes.  This condition exposed employees performing to fall hazards of up to 20 feet.  JD Construction & Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1278381, citation number 1, Item 2, issued February 26th,  2017 and became a final order on March 26th, 2018, with respect to a workplace located at  9314 E. 105th Ave., Henderson, CO 80640.  JD Construction & Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1338208, citation number 2, Item 1, issued August 14th, 2018 and became a final order on September 27th, 2018, with respect to a workplace located at 5567 Sage Leaf Court, Brighton, CO 80602.  JD Construction & Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1338214, citation number 1, Item 1, issued August 14th, 2018 and became a final order on September 27th, 2018, with respect to a workplace located at 7421 E 138th Ave. Thornton, CO 80233.
