On or about October 1, 2018, at the construction site located at Lot 60 Wellington Park, Pickerington, Ohio, an employee was installing weather barrier to the exterior of the home from a second story window opening at approximately 15 feet in height with no fall protection, thereby exposing the employee to a fall hazard.
