29 CFR 1926.20(b)(2):Accident prevention responsibilities. It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. A.) On or about December 6, 2022, an adequate job site inspection by a competent person was not conducted in that employees were exposed to a free fall hazard greater than 6' in that the rope and rope grab personal fall arrest system was not adjusted to prevent a free fall hazard greater than 6'.

On or about December 6, 2022, employees were exposed to free fall hazard greater than 6' in that the rope and rope grab type of personal fall arrest system that was in use was not adjusted properly and an employee was exposed to a free fall of approximately 30'.
