29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) Front Lawn, Right Side of Structure - Two workers in a 45 foot long by 3 foot wide trench, ranging in depth from 5 to 9 feet were wearing hard hats while two workers were handing materials and equipment to them, on or about 10/04/2018.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).    a) Front Lawn, Right Side of Structure - Workers in a 45 foot long by 3 foot wide trench with depth ranging from 5 foot to 9 foot were not protected from cave-ins while installing water lines, on or about 10/04/2018.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Front Lawn, Right Side of Structure - The portable ladder workers used to gain entry and exit out of a 45 foot long by 3 foot wide trench, depth ranging from 5 foot to 9 foot, extended approximately 12 inches past the landing surface, on or about 10/04/2018.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
