29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  a) Construction site, Roof of unit block 13-25 - On or about October 4, 2018, employees threw cut off portions of roof sheathing from the roof more than 20 feet to the ground below, exposing employees walking/working below to serious injury from the falling debris.

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks (B) Means whereby personnel on the platform can shut off power to the truck (C) Protection from falling objects as indicated necessary by the operating conditions:  a) Construction site, rear of unit block 13-25 - On or about October 4, 2018, the employer did not take precautions to protect an employee riding on unsecured wood sheathing on the forks of a Pettibone 8044, while accessing and transferring material to the second and third floor work area.
