29 CFR 1926.20(b)(4): The employer shall permit only those employees qualified by training or experience to operate equipment and machinery:    (a) On or about 10/09/18 - 351 Overton Road, Vestavia Hills, Alabama, the forklift operator had not been trained and evaluated as required.  Prior to permitting an employee to operate a powered industrial truck, the employer shall ensure that each operator has successfully completed the training required.
