29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. Furniture Warehouse, building 4241; on or about 10 October 2018 and times prior thereto; two instances were noted where energized electrical outlets had damaged receptacles: a. Pallet sorting area adjacent to main loading dock; on or about 10 October 2018 and times prior thereto; an energized two-place wall mounted electrical outlet had visible damage to the receptacle and faceplate. b. Mattress storage area, an energized two-place wall mounted electrical outlet had visible damage to the receptacle. This condition exposed employees to electrical shock and electrical burn hazards. The Army Air Force Exchange Services was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(b)(7)(iv); which was contained in OSHA inspection number 1147792, notice number 1, item number 1, and was affirmed as a final order on 16 June 2016, with respect to a workplace located at the Joint Base Elmendorf-Richardson Army and Air Force Exchange Services Direct Operations Food Court.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. AAFES Furniture Warehouse, Building 4241; on or about 10 October 2018, and times prior thereto, two instances were noted where energized electrical openings were not effectively closed: a. Damaged rack area; a knockout plug was missing from the top of an energized 240 volt wall-mounted single position outlet. b. Rear office area; a two-position 120 volt faceplate was not properly installed on the outlet box, exposing energized wires. This condition exposed employees to electrical shock and electrical burn hazards. The Army Air Force Exchange Services was previously cited for violations of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii); which was contained in OSHA inspection number 1136291, notice number 1, item number 2, and was affirmed as a final order on 04 May 2016, with respect to a workplace located at the Eielson AFB Army and Air Force Exchange Services Building 3338 Concessions area.
