29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  JOBSITE:  The employees securing roof insulation with adhesive on the roof were not protected from falling approximately 22 feet 4 inches to the lower level through ten (10) unguarded roof openings by the use of guardrail systems, personal fall arrest systems, covers or other safe means.  On or about October 10, 2018, one (1) of two (2) employees performing the above tasks on the roof fell approximately 22 feet 4 inches to the lower level through one (1) of ten (10) roof openings and sustained fatal injuries from the incident.
