Inc. on or about October 11, 2018, at the address of 901 Cleveland Street, Elyria, Ohio: The employer failed to ensure employees were adequately protected from cave-ins while performing work on a water main at the end of the trench box in a 7 ft. deep trench.

29 CFR 1926.652(g)(2): Excavations of earth material to a level greater than 2 feet (.61m) below the bottom of a shield was permitted: A) Fabrizi Trucking & Paving Company Inc. on or about October 11, 2018, at the address of 901 Cleveland Street, Elyria, OH: The employer failed to ensure the bottom of the protective trench shield did not exceeded a depth 2ft from the base of the trench. Employees were exposed to trench wall failure when the bottom of the protective shield was 3 ft. and 7 tenths above the bottom of the trench during pipe installation.
