29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a. On or about October 10, 2018, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as change blades on equipment including, but not limited to, the #6 Wood-Mizer PD200 tear down band saw, and are exposed to amputation hazards from potential movement of the blade in that the equipment was not properly deenergized.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a.)    On or about October 10, 2018, employees were exposed to amputation injuries in that the #6 Wood-Mizer PD200 tear down band saw was not locked out prior to performing change over activities, including but not limited to thread head changes. The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to maintenance tasks; the employer did not:  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  - Verify that isolation and deenergization of the machine or equipment had been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a.)    On or about October 10, 2018, the employer did not ensure employees were protected while using the #6 Wood-Mizer PD200 tear down bandsaw. Employees were required to use the saw to break down pallets and are exposed to amputation hazards from the unguarded saw blade.

On or about October 10, 2018, the employer did not ensure the amputation that occurred on October 5, 2018, was reported to the Toledo Area Office within 24 hours of the incident.  An employee suffered amputation injuries while attempting to dump a rolling dumpster into a trailer.
