29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Main Production Studio: An employee engaged in drilling holes onto a metal frame was working at an elevated height of 7 feet above the ground surface without the use of a personal fall arrest system or other means of fall protection. The employer did not ensure that employees performing work at elevated heights above the ground surface were protected from falls through a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 10/15/18.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, and orderly: a) Hot Wire Foam Area: Flexible extension cords and cords from electric powered hand tools were strewn throughout the work floor, thus exposing employees to trip, slip and fall hazards, on or about 10/15/18.
