29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or about October 2, 2018, Lumin Systems, Inc., as the controlling contractor, failed to ensure Ian Roofing & Remodeling, LLC employees were protected from fall hazards in excess of six above the lower working surface, while conducting reroofing activities, without the benefit of fall protection systems being in place.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about October 2, 2018, and at times prior, the controlling contractor, Lumin Systems, Inc., did not ensure Ian Roofing & Remodeling, LLC, protected their employees from construction hazards including, but not limited to falls, in that their accident prevention program did not include frequent site inspections to prevent persistent and reoccurring hazards.
