29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Lumin Systems, Inc. at1521 Scott Canyon Ln. Castle Rock, CO 80104: On or before 6/11/21 the controlling and correcting employer did not protect subcontractor employees from injury in that subcontractor employees did not wear fall protection while performing tear off activities on a 2 story single family home with a roof slope of 5/12. This condition exposed subcontractor employees to fall hazards of 9, 15, and 18 feet. Abatement Note: Abatement certification and documentation are required for this item. See enclosed "Certification of Corrective Actions Worksheet." Lumin Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1353167, citation number 1, item number 1 and was affirmed as a final order on 12/4/18, with respect to a workplace located at 1412 Sunrise Lane Colorado Springs, CO 80906.
