29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) Building 3, Building 3, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to Acetone, Hydrocarbon Propellant, Aliphatic Petroleum Distillates, Aliphatic Hydrocarbon, Dipropylene Glycol Ether, and welding fumes. The employer did not provide medical evaluations to employees required to use the respirators as required in 1910.134(c)(1)(ii).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Building 2, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to caught in hazards while utilizing an unguarded Delta drill press (serial # R 9251 and cat # 17-900). b) Building 2, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to caught in hazards while utilizing an unguarded Shop Craft drill press (model T6744-20P and mfg. # 327740). c) Building 3, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to flying chips while utilizing the Vectrax milling machine (model GS16V and serial # 821042). d) Building 3, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to flying chips while utilizing the General Electric 3 Head milling machine (serial # 5K2261560). e) Building 3, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to flying chips while utilizing the HAAS milling machine (model GRV 553 and serial # 570/571). f) Building 3, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to flying chips while utilizing the Bridgeport milling machine (Series I and serial # 74873-2).

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) Building 2, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to the unexpected release of energy while performing maintenance on equipment, such as, but not limited to, the HAAS 2002 CNC Machine (model VF-2D and serial # 28281). The employer failed to conduct annual audits to identify inconsistencies and deficiencies as well as the methods to control them. b) Building 3, 200 Arden Drive, Belgrade, MT 59714: On or about October 11, 2018 and at times prior, employees were exposed to the unexpected release of energy while performing maintenance on equipment, such as, but not limited to, the Piranha shear machine. The employer failed to conduct annual audits to identify inconsistencies and deficiencies as well as the methods to control them.
